Gap funding made available through the PCCC

It can aid those with disorders of the brain and their caregivers

STANDARD — The Putnam County Community Center has funding available for persons with Alzheimer’s disease, dementia or a related disorder and/or their primary caregivers for the 2020 fiscal year (October 2019-September 2020).

Funds are to be used for services and supports that enhance their ability to live in the community. This includes things such as adult day care, behavioral health assessments and intervention, home health care, respite care, companion care, short-term care in health facilities, wanderer alarm systems, wanderer identification and location services and other gap filling needs (goods and supplies).

Eligible caregivers are those who are responsible for directing and managing the care of a person with dementia. This definition refers to informal caregivers such as family or friends, not paid health care professionals.
